• In order to use the Medical Group’s Data Mart, you need to be given access—which we set up.In order to get access, you need to take the 2 day Cognos training.
• New users MUST take the first day’s morning session
• In order to take the training, you need to register on-line via our website. 
• Upon completion of the (and our receiving your) on-line registration, you will be sent the Medical Group’s Data Mart User Account/HIPAA form which you will have to complete (with supervisor’s signature/approval) and return by email to Maria Lourdes Imasa.
• Attend the Cognos training.
• Your access will be granted upon completion of the training.
• NOTE:  you MUST have a UCSF email address—i.e. your email address MUST end with “@ucsf.edu.”

Training classes teach users the basics of the Faculty Practice Organization DART tools, and provide a general overview of the content available for reporting.
